Remember you must meet your deductible plus your out - of - pocket cost (if your plan calls for one)before your plan pays 100%. Also make sure the hospital your doctor sends you to is in the network. If you live out of state and no hospital is in the network call Anthem directly to see what your options are (most of the time the plan will cover as in network)

If you stil have a Medco card please use the Medco prescription forms. If you have the new Express Scripts card use that form for prescriptions. Print forms and hand to your Doctor to file out and fax.
